Suffolk Safeguarding Partnership All-Age Review LCSPR/SAR Summary- ‘Colin’ and ‘Raffy’
This all-age review was commissioned by Suffolk Safeguarding Partnership (SSP) following the death of a parent ‘Colin’ who was cared for by his teenage child ‘Raffy’. The purpose of the Child Safeguarding Practice Review (CSPR) is to examine the family’s history, professional involvement, and missed opportunities to safeguard both the child and adult. The review drew upon multi-agency records, professional reflections, and family input, aiming to identify lessons for future practice. The names Colin and Raffy are pseudonyms used to protect the identity of the family.
