Ralph

Ralph was 17-years-old when he died by suspected suicide in 2025. At the time, Ralph was living in a residential home, with plans for a transition to more independent living when he turned 18. The case did not meet the threshold for a national or local child safeguarding practice review, however a rapid review was conducted by the Suffolk Safeguarding Partnership. No definitive reasons or triggers for Ralph’s death were identified, but several areas for further exploration and learning were identified. This overview drew upon multi-agency records and professional reflections, with the aim of identifying lessons for future practice.
