‍ ‍‘Tessa’ - Learning from Lives

Following a LeDeR, Adult Social Care and Suffolk and North East Essex Integrated Care Board representatives worked together to ensure that the learning from Tessa’s situation was assimilated and disseminated.

The below learning briefing provides an overview of who Tessa was and what happened to her in the last few months of her life.

Importantly the briefing highlights the learning from the review and the main practice points for professionals.

The learning briefing is intended to support reflective practice, inform professional decision‑making, and strengthen how organisations work together to prevent future harm.

The Suffolk Safeguarding Partnership is committed to identifying, sharing, and acting on learning from all types of review across the safeguarding system, where this will strengthen practice and improve outcomes.

“If we do not create space to learn, we risk repeating harm.” https://www.anncrafttrust.org/
